Google has never released a Forms app for iOS. You can use the mobile web version in Safari, request the desktop site, or use a third-party native client.

Google publishes iOS apps for Docs, Sheets, Slides and Drive — but not for Forms. There has never been one. On an iPhone your options are the mobile web version or a third-party client.
docs.google.com/forms works in Safari, and for small edits it is fine. In practice it is a desktop interface on a phone screen: drag-to-reorder is fiddly, the settings panels are cramped, and response charts are hard to read. It works; it is not pleasant.
Whatever you use, the forms stay in your own Google account. A third-party client works through Google's official APIs — it does not hold a copy of your forms, and you can revoke its access at any time from your Google account permissions page.
If you would rather stay in Safari, aA → Request Desktop Website gives the full editor. On an iPhone screen it is cramped, but on an iPad in landscape it is genuinely workable.
